Clinical utility

Some hematopoietic cell apheresis requires plasma reduction. This process may be performed when:

1. A product must be frozen: to reduce the volume and thus the amount of space and *dimethyl sulfoxide* (DMSO).

2. There is minor ABO incompatibility with isohemagglutinins above a safety threshold.

3. There is an excess volume of a product (> 20 ml/kg patient).

Method

The volume reduction service carried out by the Cell Therapy Service (Cell Therapy Laboratory) is automated and performed in a closed system to reduce the time and risk of contamination.

The process consists of centrifugation and separation of the cellular components, guaranteeing the preservation of the cell layer. The amount of plasma removal will vary depending on clinical needs.

Reference values

The loss of cellularity of the volume reduction process must be less than 10%.

Product: HPC/MNC by apheresis.

Tube: not applicable.

Volume: less than 200 ml.

Description of the test: applies when the progenitor cell donor has a high titer of specific isohemagglutinins against the recipient's blood group (minor incompatibility). In this case, the processing laboratory performs depletion of the residual plasma in a validated procedure (fee BT104). It is also applicable if it is necessary to perform volume reduction of the HPC product prior to infusion.
